We work alongside you to help you understand your plan, connect with the right supports, and make confident, informed decisions.

This may include:

• Understanding and using your NDIS plan effectively
• Connecting with suitable, reliable providers
• Coordinating supports to work together
• Problem-solving when things aren’t working
• Supporting plan reviews and changes

Our focus is on making things feel manageable and structured, not overwhelming.

When things aren’t working — we step in

Support Coordination isn’t always straightforward.

We often work with participants and families when:

• supports have broken down
• providers are inconsistent or unresponsive
• funding isn’t being used effectively
• things feel unclear or difficult to manage

In these situations, we take a more active role — helping stabilise supports and create a clear path forward.

A more responsive approach

We are intentionally a small provider, which allows us to:

• respond quickly
• communicate clearly
• provide consistent, ongoing support

You won’t be left chasing updates or wondering what’s happening — we stay actively involved and work alongside you.

Limited availability

To maintain quality and responsiveness, Support Coordination is offered to a limited number of participants.

This ensures each person receives the time, attention and follow-through they deserve.
